  • The Personal Touch Package: 3 - 5.75 hours of care each week
  • The Perfect Balance Package: 6 - 17.75 hours of care each week
  • The Ultimate Package: 18 - 25 hours of care each week

How do I know what is the best “Health Care Package” for me?

  • WE HELP YOU TO MAKE AN INFORMED DESCISION.  An ALSA registered nurse will meet with you and ask you what you feel would be helpful in relation to your health and your personal care. If you wish, family may also be involved in setting up your plan of care. 
  • WE CONSULT YOUR PHYSICIAN. The ALSA nurse will consult with your primary care physician in regard to your medication regime, the frequency of your medications and any other treatments that may be recommended.
  • WE ADJUST YOUR CARE AS YOUR NEED CHANGES. Your initial plan of care will be set up and can easily be adjusted to increase or decrease assisted living services as your needs change.
  • WE VISIT YOU REGULARLY.  The ALSA nurse will meet with you at least every 120 days (four months) to confer with you and evaluate your needs to assure your plan of care is meeting your needs.

Why should I choose to have nurse’s aide services from Hebrew Community Services Assisted Living Services Agency (ALSA) rather than hiring a private nurse’s aide?

  • YOU HAVE PRIVACY.  ALSA staff offer frequent brief visits throughout the day, evening and night providing the care in your home only when you need it, allowing you privacy when you do not.
  • YOU HAVE CUSTOMIZED CARE.  Your schedule of care is customized to meet your need, not the agency’s staffing availability.  ALSA staff is located at Hoffman Summerwood Community 24 hours a day, 7 days a week, so we can be there when you need us, day or night.
  • YOU HAVE A CHOICE OF AN ARRAY OF SERVICES.  A continuum of care is available should you require additional health care assistance. The ALSA works very closely with the Hoffman SummerWood staff, the Hebrew Home Care Agency, Out-Patient Physical Therapy (available on-site at SummerWood), Sub-Acute Unit and The Hospital at Hebrew Health Care. Your primary care physician is considered an integral member of the team and is consulted frequently.
  • YOU ARE NEVER WITHOUT THE CARE YOU NEED.  The ALSA does not “call out sick”. There is no need to worry about being alone or having to ask family to step in to provide scheduled care.
  • YOU HAVE AFFORDABLE CARE OPTIONS.  Our specially prepared care packages are set up to be affordable and flexible. Your care can easily be adjusted as your health needs increase or decrease.
  • YOU HAVE A SENSE OF AUTONOMY.  ALSA services are provided intermittently throughout the day.  This allows a client to pursue personal interests, enjoy more privacy in the apartment, and engage in social activities.
  • YOU AND YOUR FAMILY HAVE INVOLVEMENT.  A written plan of care is created by you, the ALSA registered nurse and your family. Each staff member follows this written plan of care that you helped to create, so you are in control of the care you receive.
  • YOU HAVE ACCESS TO SERVICES 24 HOURS.  A registered nurse is on-site during business hours to supervise and support the ALSA aides.  At all other times, a registered nurse is on-call and can be reached quickly if your ALSA aide has a question or a concern to report. The on-call nurse is an ALSA employee and is well known to the aide staff and the staff at Hoffman SummerWood.
  • YOU HAVE KNOWLEGEABLE EMERGENCY ASSISTANCE.  Each ALSA nurse has access to your medical history, emergency contacts and a current list of your medications so they are able to respond quickly to your needs in an emergency.
  • YOU HAVE CARE PROVIDED BY HIGHLY TRAINED STAFF.  ALSA aides receive a minimum of 12 hours of continuous training each year. Topics of in-services include changes in health care, client’s rights, responding to emergencies and how to provide safe, quality care.
  • YOU HAVE CARE PROVIDED BY STAFF WHO ARE CLOSELY SUPERVISED.  ALSA aides are supervised regularly on-site by the ALSA nurse. This assures that the care being provided is delivered in a safe and appropriate manner as directed by a registered nurse.
  • YOU HAVE NO GRATUITIES TO PAY.  ALSA nurses and aides are appropriately compensated for their hard work. Clients do not provide gratuity or gifts to receive quality, compassionate care.
  • YOU HAVE PRIVATE COMPANION SERVICES AVAILABLE TO YOU. When additional services are desired, the level of service can often times be delivered by a private companion, at a substantial savings to the client and family.
